Rotator Cuff Tears: When Repair Helps and When Rehab Is Enough
Rotator cuff tears are among the most common causes of shoulder pain and disability. By age 60, roughly 25% of adults have a rotator cuff tear, many without knowing it. The decision between surgical repair and structured rehabilitation depends on tear characteristics, symptoms and patient goals.
Rotator Cuff Anatomy
The rotator cuff is a group of four muscles and their tendons that stabilize the shoulder joint and control rotation:

The Evidence: Surgery vs Rehabilitation
For partial-thickness tears and small full-thickness tears without significant weakness, the evidence supports starting with rehabilitation:
- The CUFF trial (Kuhn et al., 2013) randomized patients with atraumatic full-thickness tears to surgery or PT, finding no significant difference in outcomes at 2 years. However, 24% of the PT group crossed over to surgery.
- A Cochrane review (2019) concluded that for small to medium atraumatic tears, surgery and conservative management produce similar functional outcomes at 1 to 2 years.
When Surgery Is Favored
- Acute traumatic tears: A sudden injury causing a previously normal tendon to tear (falling on an outstretched arm, shoulder dislocation) has better surgical outcomes than waiting, because the tendon and muscle quality are typically good.
- Significant weakness: If you cannot raise your arm above shoulder level or have lost meaningful external rotation strength, the torn tendon is likely the functional problem, not just a pain generator.
- Large or massive tears at risk of progression: Tears involving the infraspinatus or multiple tendons may become irrepairable if left too long due to muscle atrophy and fatty infiltration (Goutallier classification).
- Failed rehabilitation: Patients who complete 3 to 6 months of quality PT without meaningful improvement are reasonable surgical candidates.
When Rehabilitation Is Reasonable
- Partial-thickness tears: The tendon still has structural attachment. PT strengthens the remaining cuff and surrounding muscles to compensate.
- Small degenerative tears in patients over 60: Many of these tears are incidental findings on MRI that do not correlate with the patient's symptoms.
- Low-demand patients: If the patient does not need overhead strength for work or sport, rehabilitation often provides sufficient pain relief and function.
Rehabilitation Protocol
Effective rotator cuff rehab takes 8 to 16 weeks and progresses through three phases:

Post-Surgical Recovery Milestones (Weeks)
The biological healing of tendon to bone takes approximately 12 weeks. Strengthening is delayed until this healing is sufficiently advanced to withstand load. This is why rotator cuff repair recovery is significantly slower than meniscus surgery or ACL reconstruction.
Retear Rates and Realistic Expectations
