The shoulder is the most mobile joint in the body, which makes it both remarkably useful and remarkably vulnerable. Unlike the hip and knee, which are deep ball-and-socket joints held together by strong ligaments, the shoulder sacrifices stability for range of motion. A shallow socket, a ring of cartilage (labrum), and a group of four muscles and tendons (the rotator cuff) do most of the work of keeping your arm attached and functional.
When these structures fail, whether from acute injury, chronic wear, or arthritis, the surgical options range from minimally invasive arthroscopic repairs to full joint replacement. This guide covers the most common shoulder surgeries, who they are for, and what recovery looks like.
Rotator cuff repair
The rotator cuff consists of four muscles (supraspinatus, infraspinatus, teres minor, and subscapularis) that stabilize the shoulder and enable overhead motion. Tears can occur from a single traumatic event (a fall, a forceful pull) or from gradual degeneration over years of use. Partial tears may respond to physical therapy and injections. Complete tears in active patients typically require surgical repair.
Most rotator cuff repairs are performed arthroscopically through small incisions, using suture anchors to reattach the torn tendon to bone. The procedure takes one to two hours. Recovery is prolonged compared to other shoulder procedures: the arm is in a sling for four to six weeks, active motion begins at six weeks, strengthening at twelve weeks, and full recovery takes four to six months. The tendon-to-bone healing is the limiting factor; the repair is vulnerable during the first three months.
Success rates for rotator cuff repair depend heavily on tear size, tendon quality, and patient factors. Small to medium tears have healing rates above 85%. Large and massive tears have lower healing rates but often still improve pain and function even when the repair does not fully heal.
Labral repair (SLAP and Bankart)
The labrum is a ring of fibrocartilage that deepens the shoulder socket and serves as an attachment point for the biceps tendon and several ligaments. Labral tears come in several varieties. SLAP tears (Superior Labrum Anterior to Posterior) affect the top of the labrum where the biceps tendon attaches. Bankart lesions affect the front-bottom of the labrum and are associated with shoulder dislocations.
Labral repair is performed arthroscopically, reattaching the torn labrum to the socket rim with suture anchors. Recovery involves four to six weeks in a sling, gradual return to motion, and four to six months before return to sports. In patients over 40, SLAP repair is increasingly being replaced by biceps tenodesis (reattaching the biceps tendon to a different location), which has comparable outcomes with a simpler recovery.
Total shoulder replacement
Anatomic total shoulder replacement replaces both the ball (humeral head) and socket (glenoid) with prosthetic components. It is indicated for end-stage shoulder arthritis in patients with an intact rotator cuff. The procedure requires the rotator cuff to function properly because the muscles provide the active motion and stability of the reconstructed joint.
Recovery involves a sling for four to six weeks, passive range of motion starting at two weeks, active motion at six weeks, and strengthening at twelve weeks. Most patients achieve functional overhead motion by four to six months, though continued improvement occurs through one year. Pain relief is typically excellent: most patients describe the shoulder as dramatically better than before surgery within the first few months.
Reverse shoulder replacement
Reverse total shoulder replacement is one of the most important orthopedic innovations of the past two decades. It reverses the normal ball-and-socket anatomy: the ball is placed on the socket side and the socket is placed on the arm bone side. This design shifts the center of rotation, allowing the deltoid muscle to power the arm even when the rotator cuff is torn or absent.
Reverse replacement is indicated for patients with rotator cuff tear arthropathy (arthritis combined with a massive, irreparable rotator cuff tear), failed anatomic shoulder replacement, complex fractures in elderly patients, and certain revision situations. It is increasingly used for primary arthritis in older patients due to its predictable outcomes.
Recovery follows a similar timeline to anatomic replacement. Overhead reach may be somewhat more limited than with an anatomic replacement, but pain relief and functional improvement are consistently strong.
